Medical forms and guidelines

Fully up-to-date immunization records or a valid, signed exemption form need to be turned in and verified PRIOR to the first day of school.

Personal/Religious Exemption Form

• If your child has any health conditions or medications that will need to be addressed at school, please consult with our school nurse or secretary as soon as possible.
• If your child has a life-threatening allergy, please complete the Allergy Action Plan as well as a Medication Authorization form.
• If your child has asthma, please complete the Asthma Action Plan as well as a Medication Authorizaton form.
• If your child does has other conditions and will need medication to be kept at school for routine or as needed use, please complete the Medication Authorization form.
• Please include any additional care plans from health care providers as they pertain to your student’s condition.
• These forms as well as medications need to be supplied to the school PRIOR to your student attending on the first day.
